Birth and death certificates in Kolkata are issued through the Janma Mrityu Tathya Portal of the Government of West Bengal. For an event occurring within Kolkata Municipal Corporation (KMC) jurisdiction, the event must be registered with KMC before a certificate can be obtained.
Birth Registration in Kolkata
Birth in a Private Hospital or Nursing Home
Private hospitals and nursing homes must report the birth to the concerned Borough Executive Health Officer within the prescribed period. Facilities having access to the Janma Mrityu Tathya Portal can enter the required information directly through the portal.
Parents may receive an SMS link to provide the child’s name and subsequently download the birth certificate.
Birth in a Government Hospital
Birth certificates can be obtained through the concerned government hospital using the Janma Mrityu Tathya Portal.
Home/Domiciliary Birth
A domiciliary birth should be reported to the concerned Ward Health Unit, along with the required medical certificate. Physical verification may be conducted before the certificate is issued.
Death Registration in Kolkata
Where a death occurring within KMC jurisdiction is followed by cremation or burial at a KMC facility, the death event is registered at the concerned crematorium or burial ground, and the certificate can be issued through the portal.
The prescribed Certificate of Death (Form 4/4A) is required for registration.
Where the death occurred within KMC but the body is disposed of outside KMC jurisdiction, registration is handled by the concerned KMC Borough where the death occurred, subject to the required death and cremation documents.
Delayed Birth & Death Registration
Under the information provided by KMC, birth or death events are required to be registered within one year of occurrence.
For registration after one year, an order from the Executive/1st Class Magistrate of the KMC area is required.
